Out-of-state Medicaid: Accepts pending pre-authorization from the Medicaid provider. There are financial assistance programs for those living in Indiana: Dearborn county; Kentucky: Boone, Campbell and Kenton counties; and Ohio: Butler, Clermont, Hamilton and Warren counties. Ohio residents may qualify for the H-Cap program, which is for low income and based on federal poverty guidelines.
